Provides counseling, resource and subsidy information and makes referrals related to prenatal, children, child care and family services.

SALARY:
$2,770.00 Monthly

At least one (1) year experience working in human services or public health and graduation from an accredited college or university with a Bachelor's Degree in Public Health, Human Services, Sociology or related field OR, A minimum of three (3) years experience working in public health, human services or child development and a Associate's Degree in Human Services, Child Development, Sociology or related field, (PROOF OF EDUCATION IS REQUIRED.)
1.Must possess a valid Tennessee driver's license or secure one as of date of employment.
2.Must have daily access to an operable automobile and meet county requirements of automobile insurance as of date of employment.
1.Provides case management services to client base.
2.Makes referrals for appropriate social service agencies and programs.
3.Tracks referrals for completion.
4.Performs data entry relevant to family caseload.
5.Attends meetings relevant to service delivery and staff development, and works in collaboration with other agencies.
6.Performs related responsibilities as required or directed.
1.Excellent verbal and written communication skills.
2.Demonstrated computer competency in various computer applications and database.
3.Highly sophisticated organizational skills; ability to multi-task with ease.
4.Ability to provide excellent customer service and work effectively with a diverse clientele.
